Direct Primary Care is a partner or alternative to conventional insurance.  Patients pay a monthly fee in exchange for a range of basic, primary, and diagnostic care.  This benefits both the patient and the physician.  Patients are provided with regular care and screenings at a lower cost than traditional insurance fees while eliminating the insurance billing waits for physicians which in turn allows them to focus more time on patient care.

We cap a family’s membership costs at $200 per month.  This includes two parents and three children living at the same home or attending college.  Any additional children in the family are free.  Registration is $50 per family up to five family members.  Membership is billed at the beginning of each month.  The first month’s membership dues will be invoiced on the first day of each month.

OTHER DETAILS
Membership contracts are month-to-month with a 30-day cancellation requirement. If you notify us more than 30 days prior to your monthly billing date, then that will be the last payment due.  If you notify us of cancellation within 30  days of your next billing date, then there will two additional billing dates.

MEMBERSHIP PRICING:
Starting January 1st, 2018 DPC membership pricing will be:
• School-age children to 18 years of age are $25 per month.
• Young adults 19-39 years of age are $45 per month.
• Adults 40-64 years of age are $55 per month.
• Seniors 65 years of age and older are $65 per month.

The Basic Membership includes the following:

• • • • Up to 2 Acute and/or Non-acute Office visits per month.
      • Urinalysis
      • Urine pregnancy test
      • Drug test
      • Tuberculosis skin test
      • Suture removal
      • Electrocardiogram
      • Ear wax removal
      • Visual acuity screening
      • Drawing basic labs
      • Escribing medications
      • Labs and testing that cannot be performed in-house will be offered at a discounted rate through select vendors.
      • ** The patient is responsible for all fees associated with any outside laboratory testing, pathology, and specimen analysis.
